You can submit an application for a certificate of conformity using your e-identity.

After logging in, some of your personal data will be transferred to the form automatically, while other data must be entered manually and the required attachments must be added. You can also save the application in your eGovernment account.

Send the application and supporting documents by post to: Ministry of Health, Štefanova ulica 5, 1000 Ljubljana, or by email to gp.mz@gov.si.

Completed applications must also be accompanied by proof of payment of the administrative fee of EUR 4.50 to the account of the Ministry of Health of the Republic of Slovenia, Štefanova 5, 1000 Ljubljana, sub-account JFP, account number 01100 - 1000315637 and reference number 11 27111 - 7111002-21 (for payments from abroad SWIFT: BSLJSI2X, IBAN: SI56 01100-1000315637, delivery account 11 27111 -7111002-21).
