In addition to the address for service (which is the address of the individual’s permanent or temporary residence, consisting of the municipality, town, street with house number and suffix, and flat number), the individual has the option of designating an electronic address for service. The electronic address for service can consist of only one email address for service and one mobile phone number for service.

After logging on with your e-identity, your personal data are entered automatically in the form. Complete the form with accurate data for electronic service (email address and mobile phone number).

You can submit an application for the designation of an electronic address for service at any administrative unit in Slovenia or any Slovenian diplomatic mission or consulate abroad.
The form is filled in by the clerk. It is your responsibility to review the completed form and, if you agree with it, to sign it. You must provide accurate data (email address and mobile phone number).
