Obtaining a Statement of awareness with the quarantine at home order due to arrival from a high-risk area for infection with the causative agent of COVID-19 infectious disease or due to high-risk contact with person, who has a confirmed infection with the causative agent of COVID-19 infectious disease.

A person who has come from abroad (from a high-risk area for infection with the causative agent of COVID-19 infectious disease) or has been in high-risk contact with person, who has a confirmed infection with the causative agent of COVID-19 infectious disease, remains at the address of residence and limit physical contact with persons not residing at this address for a period of seven days.

Statement of awareness with the quarantine at home order – without using e-identity

You are submitting the application without using your e-identity (digital certificate or smsPASS). You will receive a Statement of awareness with the quarantine at home order on the e-mail address you enter in application. If you do not receive the Statement within a few minutes, you have entered the wrong email address and must resubmit your application.
