After you register with your digital identity, personal data is automatically transferred to the form. In the application you must provide your new temporary address and attach proof of right to reside at the address being registered.
You can submit your application at the administrative unit where you are registering your temporary residence.

Evidence of the right to reside at the address being registered. A statement of the individual's ownership or co-ownership, a lease or sublease agreement, or the consent of the owner, a co-owner or accommodation provider, proof of the right to usufruct, use or easement of the dwelling, proof of building title and proof of the right to provision for an older person are considered to be evidence that the individual has the right to reside at the address. Evidence is not required when an individual without the capacity to contract is being registered at the address of his or her parents or other legal guardian.
